It's not immediately obvious how to create multilevel numbered headers in Word. Adding the standard numbered list formatting to a header works for that particular header level, but fails to number subheaders appropriately.
It seems the best way to go about numbering headers is first, to use the built-in styles (customised to your liking) rather than formatting headers and body text manually.
The key is to format the headers with multilevel list formatting rather than the standard numbered list formatting.
That magically formats the headers and subheaders the way you want it.
